Smile Shares Terms & ConditionsTerms and Conditions of the Smile Shares Program
SharesThe advertised fee paid for membership in the Smile Shares Program is for 12 months from the date membership is purchased and expires on the same calendar date the following year. The Smile Shares membership is a stand-alone dental plan for people without other dental benefits and cannot be used in conjunction with other dental benefit plans.
Share types can be upgraded at any time during the year by paying the additional cost of the higher annual membership fee. The cost will not be prorated and the benefits are not retroactive, only moving forward toward the original expiration date.
For new patients: after your initial comprehensive assessment, if we do not feel that we are able to provide your preventive care needs or significant portions of your treatment, we will refund your membership fee and charge you $99 for the comprehensive new patient exam and up to $120 for any x-rays taken. We will gladly provide a digital copy of your x-rays to any new provider of your choosing.

Right to Revoke MembershipStrong & Healthy Smiles reserves the right to revoke membership privileges at any time for abusive or inappropriate conduct by members toward the doctor, Smile Team members, or other patients or visitors. No refund will be made, and such people will be dismissed from the practice with a single written notice. |
CommunicationSmile Shares members agree to communicate by phone, by e-mail to drsue@strongandhealthysmiles.com, or in writing if they have any questions or concerns regarding treatment recommended or provided, service provided by Dr. Keller or Smile Team members, or suggestions to improve our service. If you have a concern, we want to know about it! We will do our best to resolve the issue.

Keeping AppointmentsStrong & Healthy Smiles requires all patients to provide 48 hours notice to change a scheduled appointment. Failure to do so incurs a penalty. For example, if your appointment is reserved for 9 a.m. on Monday, you need to call and give notice before 9 a.m. Thursday morning to change your appointment without penalty. The penalty for Smile Shares members who fail to provide sufficient notice is:
- forfeiting a visit from your plan if a cleaning or hygienist-scheduled visit is missed, or
- losing the 10-, 15-, or 20-percent courtesy from a doctor visit when it is rescheduled, or
- a charge of $99 per hour (minimum) of scheduled time as a missed appointment fee.
Please call as soon as you know of your need to change your scheduled appointment to avoid any penalties.
- Emergencies and illnesses are of course accepted
- Repeated failure to keep appointments will result in a written warning. Two written warnings may result in dismissal from the practice, with no refund of the membership fee.
Strong & Healthy Smiles will likewise give you as much notice as possible if we need to reschedule your appointment. We will also do our best to contact you if we are running significantly late on the day of your appointment and give you the option to reschedule.
It is the nature of dental treatment that sometimes we have patients needing emergency treatment or planned treatment becomes more complex or takes longer than originally anticipated. Your understanding and flexibility is appreciated.
